I’m 58 with many physical problems. I want to retire from my job and apply for ssdi. How do I succeed?

I have spinal stenosis, diabetes, no support in the ankles which causes my feet to turn or roll at the slightest lose of footing, no strength in my left leg, must use a cane to walk with,no balance which has caused me to fall on several occasions, have had to back surgeries but still am getting more and more less mobile.
